Registration
Site registration is not required. However, if you wish to post on the message boards, there is a seperate registration at the boards. Just click on the "Message Boards" link in the Main Menu above and then click on the "Please register a user name" link which is located at the top right hand side of the board.

Civil Cover Sheet

The United States District Court for the Western District of New York requires a Civil Cover Sheet to be filed with the Complaint. You can see a copy of what I filed here. The scanned images are very large, so it may take some time to load if you have a slow connection.

(Oh, and by the way...I was not aware I needed TWO copies of the Civil Cover sheet, so I had only typed one. Fortunately I had my handwritten template with me, so they stamped that one for my copy and that is what you are seeing. The Court only keeps one copy, but if you need more, be sure to take extra with you. I would recommend three copies of everything, plus the original. That way you have a copy for your files, a copy to have served, and a copy to mark up.)

The filed copy also more clearly stated my "causes of action". I had three.
